Contact your insurance company to see if they reimburse for independent Massage Therapy. If they do, ask them if you need a prescription from your doctor. If they say yes, ask them how the prescription should read. Ask them what is the code that they use. (You may need this for your claim form). Contact your doctor and get a prescription written with the terminology recommended by the insurance company.  After you pay me, I will give you a written receipt. Take the prescription from your doctor, my receipt and print and fill out a claim form from your insurance company. Send that information to your insurance company.
