Anne Clay only works by referral from Physician, Physician Assistant, Physical Therapists, Physical Therapist Assistants (PTA's) or Occupational Therapists. Please contact me at (214) 436-9956 if you need Professional References and/or Customer References from me or need my email address.

• I do not require a prescription from a Physician, but your patient may want one to file insurance on their own.
• I do not need copies of MRI's or xray's.however written reports and findings from tests are welcome
• I do not file insurance nor do I take Medicare.
• Your patient can verbally tell me on the telephone that they were referred by you and list your information on my Client Consultation Form.
• My office is not set up to work with people in wheel chairs. If a person uses a cane or rollator they must be able to get on and off the table without assistance from me.
